We’re thrilled to welcome you to our practice! We kindly request that you complete your new patient forms prior to your appointment. For your convenience, you can click on the forms below, or if you prefer, you can complete them in our office by arriving 15 minutes early. Thank you, and we look forward to seeing you soon!

Dental Insurance

Our team will gladly help you maximize your insurance reimbursements and send a monthly statement regarding your account. Remember that insurance companies will typically respond within four to six weeks, so please call our office at (916) 483-8182 if your insurance payment has not appeared on your statement. We provide estimates for all planned procedures and collect co-pays at the time of service. The remaining balance(s) after insurance payment will be your responsibility, and we thank you in advance for your understanding and cooperation. Options for a monthly payment plan are available, but they must be arranged prior to your procedure or through CareCredit.

Please note that patients are responsible for payment of their fees, regardless of insurance coverage. Of course, we are happy to answer any questions you have about your account, and we'll always do our best to ensure you receive the dental care you need!
